Athletics Move to Digital Forms
We ask ALL athletes’ parents to use FinalForms in place of our physical paper packets. You must complete registration prior to your student-athlete practicing with a team for the 2024-2025 school year. Please register at the link below, and follow the prompts to create your account, create your students and sign your forms:

**If you have already completed and turned in the packet, we will update/upload the actual signed physical once you’ve initiated your account. You will only have to complete your student’s registration one time as the information rolls over to the following year. Once the registration is complete, your student-athlete’s name will be highlighted in green. The name appears with a red highlight if information or signatures are missing. We apologize for the additional step to documenting your student-athlete’s eligibility; it will greatly shorten the registration process in the long run.
